Information Needed for Attending the Practice You are very welcome to St. George’s Dental Practice. The following information may be of assistance to you. We have a ground floor surgery and we can provide full access to dental services for anyone with restricted mobility. If you need an urgent appointment please call us as early as possible in the day. Our Practice policy is to try to see emergencies as soon as we can and on the day that you ring if at all possible. Please try to give as much notice as possible when cancelling an appointment. We usually require 24 hours notice. When you miss an appointment it wastes our staffs’ valuable time and deprives another person of the chance to be seen. If the situation should arise where two or more dental appointments are missed without prior notice to the practice we will not be able to make any more appointments for you. Please inform reception of any changes to your name, address or telephone number. Please ask at reception if you need to speak to us in private. We can provide you with a confidential place to talk. When you first attend the practice you will be asked to fill in a confidential medical history form. It is very important that you fill this in very carefully as it can affect how we plan your treatment. If you are unsure whether to put something on the form please write it in or mention it to your dentist. If there is something that you would rather not write on the form please tell the dentist directly when you see them. On return visits, if you have a change in any medical condition. medication or have an allergy to anything please inform your dentist. Our staff are hard working and constantly busy giving help and advice. Please be patient. Abuse of any kind will not be tolerated. Unacceptable behaviour by anyone will result in the person being asked to leave the premises and not being booked in for any more appointments. Following your Oral Health Examination your dentist will discuss your treatment options with you and with your agreement you will be given a written treatment plan and estimate which will include any fees due. St. George’s Dental Practice complies with the Data Protection Act 1998 and is registered with the Information Commissioner’s Office (ICO). (www.ico.org.uk). Our registration number is Z558443X. All records remain strictly confidential. We may on occasion need to share your details for the purposes of Specialist referral. All records are kept for 11 years or if you are a child until your 25th birthday. If for any reason you need a copy of your patient records we are happy to provide them but there will be a fee. Should you wish to change dentist at any time please feel free to talk to one of our reception staff who will be happy to assist you.
